DLA medical form

Hello I'm asking this for a friend. In January she was awarded DLA low rate care due to deformities of her hands.

She asked for a reconsideration due to mobility issues and more care needs.

They sent an atos doctor to do a medical and now have removed the care altogether and no mobility either. She had today recieved a copy of the medical reported and on the front page where there is boxes for the name and signature of the doctor and the date of exam and clients nino.
The boxes are empty and she has only filled in date and nino. Should the doctor ha e signed and printed their name.

Also on the day of exam and her hands were very swollen and stiff, she wears splints daily and the doctor told her not to take splints off as she could see how swollen and sore they were.

Yet on the form she has stated that she has full function off both of her hands and no deformities. Yet her fingers are all turning and are very deformed. Had the doctor taken off the splints she would have noticed this.

Any one have any comments on the above please
