How do i switch from jsa to esa?

Hi wont post whats wrong as its on my other thread :) But wanted to know what i have to do to switch from jsa to esa.

Do i sign off jsa and call esa to make a new claim (have my doctors note) Or do i call esa first and set it up then tell jsa.
