Help with DLA forms

Firstly, thankyou for nannytoone for replying when I misposted earlier on someone's else's thread by mistake. We will definitely contact DIAL.

Other than the CAB, are there any disability support groups in West Yorkshire that are able to help fill out a DLA claim form? I've tried Googling but I'm just getting lost in the minefield of answers!

Background is that my mum has suffered from Osteoarthritis since her late 20's, now 58. She has so far had both knees replaced, along with most of the bone in her lower leg. She has the condition in both hips, shoulders, spine, wrists and both feet. She has has steroid and quite a few other injections in the past with no effect. The Pain Management consultant has had he agreement from her other consultants (currently sees four different ones) to put her forward for a formal of Radiotherapy treatment to try to destroy nerve endings and hopefully make a temporary reduction in her pain levels.

She is in constant pain despite very high levels of medication, her walk is really unsteady due to the pain and the level of pain is starting to make her sick. She is determined not to "give-in" to the illness yet as she feels she is too young to stop working, has worked from being 14. She managed to find a job closer to home to cut travel time down but although I take her and pick her up if I'm not at work at the same time, her shifts change weekly and the only way to get there is taxi. We just want to see if she can get any help via DLA, as suggested by the Consultant, to cut down on the impact of the taxi costs.

We did submit an application months ago but she was waiting for an injection treatment and they misread the medical reports and assumed that the injections would resolve the problem totally. Mum wanted to wait as she had further follow ups arranged rather than appeal at the time. Her consultants have said that even with future hip replacements, her spine neck, shoulders and feet cannot be grateful so will still have considerable pain and the conditions is degenerating quite quickly.

I do as much as I can for her but there are times when I have to be at work,dropping or picking my daughter at nursery or have appointments myself. I have tried to help her with the taxi costs but I'm on my own with my daughter, working part-time so money is limited for me so can only do so much.
