Informing DWP of changes ie getting worse!!

Hi, I have a number of health problems, the latest being Osteo Arthritis, and I am on Incapacity Benefit (not been changed to ESA yet) and was awarded DLA after having to go to Tribunal.
The problem is my mobility has got a lot worse as the arthritis has spread to my hips and spine, whereas when i claimed it was just in one knee (but I had other problems which were part of the claim)
I can barely stand for very long now, let alone walk.
My bathroom was converted to a wet room by the council as i cant get into a bath, and I have various bits of equipment and rails after an OT assessment.

My problem is, if I inform the DWP about my deteriorating mobility am i going to have to face fighting for ESA and DWP all over again, with a risk of losing my benefits altogether?

I'm not due to be assessed for PIP until 2015, so should i just leave well alone until then?

Just to add, I am on Medium Care and Low Mobility rates.
I didn't apply for the care component but the tribunal awarded it to me because they could see that i needed it.
